The transition to long-term living can be challenging for both the patient and the family. The social services department at Masonicare will support you during the process by facilitating communication, assisting with resources and planning, and helping with any requests or concerns you or your family member may have.

Masonicare offers three specialized units dedicated to memory care. These units provide a safe and secure environment for those with Alzheimer’s Disease and other dementias.
The team at Masonicare is not only dedicated to your recovery, but also to the prevention of complications and rehospitalizations once you’re back home.
You can expect ongoing evaluations to determine your need for home health or outpatient therapy after discharge. Since Masonicare provides these services, you’ll have the option of an easy transition within the continuum.
