Skilled Nursing Care at Home
If you’re just home from the hospital or struggling with a chronic medical problem, our licensed, experienced nurses are available 7 days a week. Once your doctor authorizes care, a Masonicare nurse will come to your home to do a complete home health evaluation and create a healthcare plan. Thereafter, you’ll receive comprehensive care as often as required to effectively manage your condition.
What is Skilled Nursing?
It can take many forms, but skilled nursing care is provided by licensed, experienced RNs and LPNs. For some patients, it’s post-operative care; for example, to change dressings or remove drains. Others may require medical assistance with medication, advanced wound care, or perhaps a catheter insertion. All patients receive instruction on the best self-care techniques.
Whom We Help
Specially trained nurses from Masonicare can help individuals with nursing care needs after surgery or due to a chronic or acute illness, including:
- Cardiovascular Care
- Diabetic Care
- Parkinson’s Care
- Post-surgery Recovery
- Respiratory Care
- Stroke Recovery
What to Expect
Whenever possible, you will be cared for by the same nurse. Your nurse can also deliver other in-home care (for example, therapy or wound care) and make arrangements for help with bathing or in-home monitoring. All of your home health caregivers are committed to a team-based approach by staying in touch and exchanging the latest information about your condition via computer.
- RNs and LPNs provide nursing care and nursing education for patients with acute and chronic conditions.
- As needed, nurses can add additional home care services such as physical, occupational, and speech therapists, medical social workers, and home health aides.
