Palliative care is designed to alleviate pain and discomfort, provide comfort care, and improve quality of life. Palliative care is appropriate for patients in all stages of a disease, including those being treated for curable illnesses and those living with chronic diseases, as well as patients with a terminal diagnosis or who may nearing the end of life.
Masonicare Home Health & Hospice and Masonicare Partners Home Health & Hospice provide palliative care to residents and patients throughout Connecticut. Our teams of palliative care professionals carefully assess each patient’s needs, and all treatment options are explored and evaluated on an ongoing basis. The type of palliative care administered is agreed upon by the patient, his or her physician, the primary caregiver and the interdisciplinary team. Treatment may include medication(s), along with massage therapy and relaxation techniques, if needed.
Every patient’s symptoms and personal values are given consideration during this process. The palliative care team knows that effective communication with the patient, family, and physicians will help to ensure that everyone’s needs are fully met. However, the patient’s choices and decisions regarding care are of the utmost importance and will always be honored.
Emotional support. Palliative care focuses on the entire person, not just the illness, and team members will address a patient’s social, psychological, emotional, and spiritual needs.
Family/caregiver support. Caregivers and family members often bear a great deal of stress, and the palliative care team is there to provide emotional support to them as well.
Communication and coordination. Members of our palliative care team are excellent communicators, trained to help patients and families establish care goals, make decisions, and coordinate care.
Masonicare in Connecticut brings palliative care to individuals in their homes, including assisted living communities. Medicare, Medicaid and some private insurance companies reimburse for this care under their home health benefits. Masonicare will assist patients and families by reviewing payment options and the different levels of care available.
