Masonicare Home Health & Hospice Becomes a “We Honor Veterans” Partner

Newtown, CT, January 8, 2014 ……….It may surprise many people to learn that 25 percent of those who die every year in the U.S. are Veterans. To help provide the care and support that reflect the important contributions made by these men and women, the Brookfield @ Newtown office of Masonicare Home Health & Hospice has become a national partner of We Honor Veterans, a pioneering campaign developed by the National Hospice and Palliative Care Organization in collaboration with the Department of Veterans Affairs.

Partners in the program assess their ability to serve Veterans and, using resources provided as part of the campaign, integrate best practices for providing end-of-life care to Veterans into their organization. The focus of the program is on respectful inquiry, compassionate listening, and grateful acknowledgment.

The campaign provides tiered recognition to organizations that demonstrate a systematic commitment to improving care for Veterans.  As a We Honor Veterans Partner, Masonicare Home Health & Hospice must achieve the following:

Level 1 – provide veteran-centered education for staff and volunteers and identify patients who have military experience

Level 2 – build the organization’s capacity to provide quality care for veterans

Level 3 – develop and strengthen relationships with VA medical centers and other veteran organizations

Level 4 – increase access and improve quality of care for veterans in all of the communities served

The Brookfield @ Newtown Branch of Masonicare Home Health & Hospice has completed Level 2 and is currently in the process of completing Level 3.
