Masonicare Home Health & Hospice’s Dementia Program “Living with Memory Loss” is designed to assist families and caregivers of individuals with dementia cope with all the daily concerns of confusion, wandering, lack of interest in eating, and inability to complete normal activities of daily living.
The patient is the person with dementia as well as his caregiver. This program’s focus is to facilitate independence, connect caregivers with community supports and allow individuals with dementia to remain at home for as long as safely possible. Masonicare Home Health & Hospice recognizes that resources for caregivers patients with dementia are difficult to find. Our goal is to provide guidance, education, and future planning to allow patients to stay at home and for caregivers to gain understanding of the disease and care needs.
Qualified clinical staff will assist caregivers to plan for and implement ways to allow for the greatest degree of comfort, independence and safety. Clinicians, including specially trained nurses, physical therapists, occupational therapists and social workers, will meet with families, assess client and caregiver needs and develop a comprehensive plan focusing first on the caregiver’s identified issues.
